Services and Support

If you are living with HIV

If you are LIVING WITH HIV, you may qualify for supportive services. These may include medical case management, assistance with HIV-related medical visits and labs, assistance with HIV-related medications, housing (rent/mortgage) and utilities assistance, food/nutritional assistance, transportation assistance to HIV-related appointments, substance and alcohol abuse therapy assistance, mental health evaluation and treatment assistance.

Medical Case Management

Medical Case Management provides services that link you to medical care and provides support to you in other areas of your life so that you are able to continue your care. At intake, a medical case manager will collect information and talk with you about your medical care, medications, your living arrangements, mental health, your household budget and anything else that may affect your health or safety. You will also discuss how HIV disease affects you and how to prevent transmission of HIV. You and your medical case manager will then develop an action plan for care based on your individual needs and goals. Medical case managers provide support to you in coordinating the services required to complete the goals. Periodically, you will update any information that may have changed and you and your case manager will develop with a new plan based on your progress and goals. Your medical case manager will continue to monitor the completion of these goals until you are able to successfully manage your care on your own.

FREE MEDICAL SERVICES

Helping you to establish continuing medical treatment to treat your HIV disease and minimizing barriers to medical treatment is our most important focus. HIV is a complex disease and requires regular visits to primary and specialty doctors. Case Managers will make you a referral to an Infectious Disease doctor who treats HIV and AIDS and a primary care doctor who treats other medical problems. In addition, we work with your doctors to coordinate your care and monitor your treatment goals. If you cannot afford the cost of your doctor visits, we will work with you to fit this into your budget and look for other programs to help you cut down some of these costs.

FREE LAB SERVICES

Lab tests are used to monitor HIV levels in your blood. Lab tests also tell you about other levels in your body like your cholesterol and blood pressure. Doctors will look at these lab results to determine the best course of treatment for you and to see if your medications are working properly. If you cannot afford the cost of your lab tests, we will work with you to fit this in your budget and we will look for other programs that may help you cut down some of these costs.

PRESCRIPTION services

Most people living with HIV and AIDS need to take medications called antiretroviral treatment to manage their HIV. Often, these medications are very expensive and have to be taken for an extended period of time. In some cases we can help with some of these costs if you are having trouble paying for your medications. While we cannot pay for each medication every month, AID Upstate can help you apply for programs that provide long-term assistance with paying for these medications such as the South Carolina AIDS Drug Assistance Program (ADAP) or Pharmaceutical Assistance Programs (PAPs) or we can refer you to Medicaid, Medicare, or private insurance programs.
