The following positions are open at AID Upstate:
Case Manager

Purpose: Manage client caseload and provide access to eligible services.

Job duties include: conduct intakes/assessments of new clients; conduct program eligibility and benefits reviews; ensure clients have access to primary and specialty medical care, as well as access to medications; document services provided; provide community referrals to assist with food, benefits, social issues and emotional support; perform home visits as needed.

Housing Case Manager
Purpose: Coordinate agency housing assistance programs.

Job duties include: develop and monitor client housing plans; manage the housing program budget; monitor documentation of housing services provided; prepare and submit housing assistance reports; prepare and submit housing payments; refer clients to outside agencies as appropriate; keep abreast of federal, state, and local housing program guidelines, procedures, policies and laws.

Bachelors degree in human services field, working knowledge of Windows-based computer programs (MS Office, etc.) an valid SC drivers license required. Two years relevant work experience preferred.
